Assisted classroom teacher in preparation of alternative academic course work for students participating in a high school-level Life Skills program.  Provided one-to-one support for students participating in mainstream classes.  Provided job-coaching support for students engaged in community-based supported employment opportunities. 

 

Licensed Social Worker

September 2005 - August 2007

Using the Wraparound©  process to garner input from various stakeholders, assisted families of children with mental health diagnoses in identifying areas of strengths and needs, then assisted with accessing resources to help meet identified needs.  Maintained confidential client records.  Compiled reports and researched client social histories, then wrote comprehensive assessments based on that research. Utilized family-friendly approach to gather information during frequent home visits.  

 

Family Independence Specialist

October 1998 - November 2000

Interviewed and screened low-income persons applying for the Food Stamps and Medicaid programs to determine applicant eligibility. Provided ongoing case management services for qualified clients and referred those clients to other community-based service organizations when appropriate.  Maintained confidential client records.

Supervised and performed clerical functions including confidential documents management. Assisted with grant procurement.
